The four numbers that matter
Total cholesterol sets the headline. HDL is protective — higher is better. LDL is the cholesterol most associated with cardiovascular risk. Triglycerides are the other major blood fat, particularly responsive to sugar, alcohol and refined carbohydrates.
The ratio is more useful than the total
Total cholesterol divided by HDL is one of the strongest single predictors of cardiovascular risk. Below 4 is considered healthy. Above 5 suggests further investigation is worthwhile. Two people with the same total cholesterol can have very different ratios and therefore very different risk profiles.
What moves the numbers
Lipid markers respond strongly to lifestyle. Reducing refined carbs and added sugar lowers triglycerides faster than reducing dietary fat does. Soluble fibre (oats, beans, lentils) lowers LDL. Strength training raises HDL. Stopping smoking improves all four markers within months. Weight loss of 5 to 10 percent of body weight produces measurable improvements in most people.
When numbers do not respond to lifestyle
Some people have inherited lipid disorders (familial hypercholesterolaemia) where lifestyle alone cannot bring numbers into target range.
