Biomarkers Before Starting GLP-1: Your Pre-Treatment Baseline

A baseline blood test before starting GLP-1 medication captures the metabolic markers most affected by treatment, so you can track real change.

GLP-1 receptor agonists like semaglutide and tirzepatide affect a wide range of biomarkers, not just body weight. A pre-treatment baseline lets you see the real metabolic impact over time, beyond what the scale shows.

Why baseline matters

Without a baseline, you have nothing to compare against. Six months in, you will not know whether your insulin sensitivity has improved by 40 percent or stayed the same. The baseline turns the treatment into something measurable.

Markers most affected by GLP-1

Fasting glucose, HbA1c and fasting insulin (often improve significantly). Lipid profile (typically improves). Liver enzymes ALT, AST, GGT (often fall as fatty liver resolves). Thyroid markers (worth monitoring in some patients). Nutrient status including B12 and iron (can drop if appetite is significantly reduced).

What about side effects?

Some side effects of GLP-1 medications include nausea and reduced appetite which can cause inadequate nutrient intake. Pre-treatment markers help identify nutrient gaps before treatment so you can plan supplementation accordingly.

Frequently asked questions

Do I need GP approval to test before starting GLP-1?

No. Anyone can order a Chxhealth blood test. The medication itself is a separate prescribing conversation with a qualified clinician.

How long after starting GLP-1 should I retest?

Three months is a common first follow-up, then every 3 to 6 months. Earlier if your clinician requests it.
